Client Rights and Responsibilities

Your rights as a recipient of services at *Behavioral Health Group are:

  1. To be treated with dignity, courtesy, and respect for your individuality.
  2. To receive personalized care based on your needs.
  3. To know the cost of services prior to receiving them.
  4. To know the identity of your provider (name, business address, contact information, specialty, etc.).
  5. To receive information about the services offered.
  6. To receive information about treatment and participate in treatment planning.
  7. To consult with other medical or service specialists when it is indicated.
  8. To seek treatment elsewhere upon request.
  9. To not have your picture taken unless you give written permission to do so.
  10. To receive services in a safe, clean, and pleasant place.
  11. To be cared for with reasonable regularity and continuity of staff assignments whenever possible.
  12. To be free from discrimination based on Race, Color, National Origin, Sex (interpreted to include gender identity, sexual orientation, and pregnancy status), Age and Disability.
  13. To be free from exploitation for the benefit or advantage of the provider.
  14. To terminate services, unless the services are court ordered.
  15. To know who will get psychological assessment results, withdraw consent to release the results (unless court ordered or required by law), and to have the results provided in understandable language.
  16. To report a complaint or grievance related to the violation of your patient's rights without fear of retaliation. Complaints may be submitted here: OBC Patient Complaint Submission
  17. To be free from physical, sexual, emotional abuse, neglect, exploitation, and retaliation.
  18. Referrals as appropriate to other community resources.
  19. Behavioral Health Group will report all instances of abuse and clear danger of suicide or homicide as required by law.

In addition to your rights as a recipient of services with Behavioral Health Group*, you can ensure the best treatment outcomes by assuming the following responsibilities:

  1. Give accurate information about present complaints, past illnesses, hospitalizations, medications, and other health-related matters to the best of your knowledge.
  2. Understand your health and participate in developing treatment goals/care plans.
  3. Follow the treatment goals/care plans that you have agreed to.
  4. Tell your provider about any changes in your health or emotional state.
  5. Inform staff if you feel as though you are unable to follow through with your treatment goals/care plan.
  6. Be considerate of other patients and family members and respect their need for privacy.
  7. Keep all scheduled appointments. If you are unable to make your appointment, cancel at least 2 business days in advance, by sending a patient portal message or calling your Behavioral Health Group. If the problem persists, patients may be discharged after three (3) missed appointments or late cancellations.
  8. For appointments cancelled less than 2 full business days in advance, a missed session fee of $20 may apply for Group therapy sessions, a fee of $100 may apply for Medication Management and Therapy appointments, and a missed session fee of $300.00 may apply for psychological/testing appointments. Note - This statement is not applicable to patients with Medicaid.
  9. Arrive on time for your appointments. Depending on how late you are and the nature of your appointment, you may be responsible for paying a late cancellation fee as noted above in item #7. Note-This statement is not applicable for patients with Medicaid
  10. Inform staff of any medical condition that may be contagious.
  11. Inform staff if you have thoughts of harming yourself or others.
  12. Take medications as prescribed (if applicable).
  13. Tell your provider if you are having side effects from your medications, or if your medication is not working.
  14. Discuss concerns with your provider if you do not agree with their recommendations regarding your treatment goals/care plans.
  15. Tell your provider when and if you want to end treatment.
  16. Inform us of any changes to your contact information or status of your health insurance
