Client Rights and Responsibilities
Your rights as a recipient of services at *Behavioral Health Group are:
- To be treated with dignity, courtesy, and respect for your individuality.
- To receive personalized care based on your needs.
- To know the cost of services prior to receiving them.
- To know the identity of your provider (name, business address, contact information, specialty, etc.).
- To receive information about the services offered.
- To receive information about treatment and participate in treatment planning.
- To consult with other medical or service specialists when it is indicated.
- To seek treatment elsewhere upon request.
- To not have your picture taken unless you give written permission to do so.
- To receive services in a safe, clean, and pleasant place.
- To be cared for with reasonable regularity and continuity of staff assignments whenever possible.
- To be free from discrimination based on Race, Color, National Origin, Sex (interpreted to include gender identity, sexual orientation, and pregnancy status), Age and Disability.
- To be free from exploitation for the benefit or advantage of the provider.
- To terminate services, unless the services are court ordered.
- To know who will get psychological assessment results, withdraw consent to release the results (unless court ordered or required by law), and to have the results provided in understandable language.
- To report a complaint or grievance related to the violation of your patient's rights without fear of retaliation. Complaints may be submitted here: OBC Patient Complaint Submission
- To be free from physical, sexual, emotional abuse, neglect, exploitation, and retaliation.
- Referrals as appropriate to other community resources.
- Behavioral Health Group will report all instances of abuse and clear danger of suicide or homicide as required by law.
In addition to your rights as a recipient of services with Behavioral Health Group*, you can ensure the best treatment outcomes by assuming the following responsibilities:
- Give accurate information about present complaints, past illnesses, hospitalizations, medications, and other health-related matters to the best of your knowledge.
- Understand your health and participate in developing treatment goals/care plans.
- Follow the treatment goals/care plans that you have agreed to.
- Tell your provider about any changes in your health or emotional state.
- Inform staff if you feel as though you are unable to follow through with your treatment goals/care plan.
- Be considerate of other patients and family members and respect their need for privacy.
- Keep all scheduled appointments. If you are unable to make your appointment, cancel at least 2 business days in advance, by sending a patient portal message or calling your Behavioral Health Group. If the problem persists, patients may be discharged after three (3) missed appointments or late cancellations.
- For appointments cancelled less than 2 full business days in advance, a missed session fee of $20 may apply for Group therapy sessions, a fee of $100 may apply for Medication Management and Therapy appointments, and a missed session fee of $300.00 may apply for psychological/testing appointments. Note - This statement is not applicable to patients with Medicaid.
- Arrive on time for your appointments. Depending on how late you are and the nature of your appointment, you may be responsible for paying a late cancellation fee as noted above in item #7. Note-This statement is not applicable for patients with Medicaid
- Inform staff of any medical condition that may be contagious.
- Inform staff if you have thoughts of harming yourself or others.
- Take medications as prescribed (if applicable).
- Tell your provider if you are having side effects from your medications, or if your medication is not working.
- Discuss concerns with your provider if you do not agree with their recommendations regarding your treatment goals/care plans.
- Tell your provider when and if you want to end treatment.
- Inform us of any changes to your contact information or status of your health insurance
